  2. Will I see you each time we have an appointment, or will others in the practice see me?
  3. Will you or others in the practice discuss health problems over the phone? Do you charge for such “telephone time?”
  4. What happens if I have an emergency outside of office hours?
  5. Are you a member of the health plan I belong to?
  6. How must I pay for care?
  7. Where will I deliver?
  8. How do you feel about me obtaining a second opinion?
  9. What is your medical training and where did you go to medical school?
  10. Are you board certified?
  11. How many babies have you delivered?
  12. What prenatal tests do you usually recommend, and why?
  13. Do you handle high-risk pregnancies (such as multiple gestations or older mothers)?
  14. What is your basic philosophy of birth?
  15. What is your cesarean section rate?
  16. How often do you induce labor, why and how?
  17. How do you handle problems or complications that may develop during labor?
